There is a problem with CRM 4.0 RTM for upgraded CRM instances. It is impossible to delete items from a queue. They seem to get half deleted and stick around in the queue in a non-functional state. This messes up your views and can quickly overwhelm anyone that is using queues to manage their work. It appears as though the source of the bug is a trigger from CRM 3 that was not deleted in the CRM 4 upgrade process. I found a blog article that prescribes the resolution to the fix (involves disabling the trigger and manually updating the records as deleted). I will update this article when I find a specific Microsoft-endorced hotfix. In the meantime, here is the article that discusses the problem and his fix:

There is a SQL script at the bottom of this page that seems to work and clean up all the activities in all the queues that were not properly deleted.
